NGA CARD

  1. Bodybuilding, Figure AND Bikini competitors must purchase an NGA Membership.
    If you need to purchase an NGA Membership card (Bodybuilding, Figure and Bikini Only) please click here.
  2. If you already have a current NGA membership you must know your current card# and expiration date to fill out the entry form. If you just purchased your NGA membership card online then put your order confirmation # and purchased date in the appropriate boxes instead.

STEP 2:

PAY ENTRY FEES Be sure to pay for a trainer’s pass now.

Pay your fees by selecting the option below. Entry fee includes pre-judging, Saturday events and night show.

STEP 3:

POLYGRAPH TEST

ALL BODYBUILDERS AND FIGURE CONTESTANT’S MUST BE POLYGRAPHED BY OUR DESIGNATED EXAMINER, JAMES PAGE or provide proof of a polygraph taken within the 90 days prior to May 5th, 2012.  (Bikini and Kids do NOT need a polygraph test) SEE SCHEDULING INFO UNDER DRUG TESTING.

WAIVER: By your accepting my application to the 2012 NGA Mountain States ‘The Show’ Championships along with my non refundable entry fee, I hereby intend to be legally bound for myself, my heirs, executors and administrators, and waive and release any and all rights and claims for damages I may have against the NGA, Destiny by Design Inc., Kelly & Laurie Moore, and any and all sponsors, agents, representatives and assigns for any and all injuries and/or losses suffered by me as a result of my participation and/or attendance and traveling to the 2012 NGA Mountain States ‘The Show’ Championships.
These damages include, but are not limited to, published photographs that I may find to be unattractive or editorial which I may construe as being misrepresentation. I also grant the NGA and NGA Mountain States ‘The Show’ Championships., Permission to use photos, videos or any likeness of myself to promote any future contests, videos, magazines or any other media involved with future and present contests for the purpose of promotion and/or sales of these media, without any compensation to myself, as condition of my entering the event. I also agree, by my own free will and without duress, to take a polygraph test and submit a urine sample. I also agree to the outcome of the tests results from the polygraph examiner and urinalysis center. I will abide by all NGA rules and show good sportsmanship. The event owners/promoters reserve the right to refuse participation to any athlete should a discrepancy arise.

By submission of this entry form I am legally stating that I am at least 7 years Drug Free and will abide by the above waiver and the NGA drug testing policies and procedures.
